Patient Portal
Please call or email in order to schedule your first appointment. Once your appointment is scheduled, you will be sent an email with information to create your TherapyNotes Portal Account. You will not be able to access the portal until you schedule an appointment and we email you instructions. We ask that you complete your new patient paperwork at least 48 hours before your appointment. If you have any questions or concerns, please contact us directly via phone or email. 

If you are an existing patient and you need your TherapyNotes Portal password reset, please contact your therapist directly to receive a reset link.
